Appointment of Agent to Control Disposition of Remains

This form permits the designation of a representative to carry out all funeral arrangements, including method of disposition (i.e.- burial, cremation). The person filling out this form should retain a copy as well as mail a copy to the funeral home. Agency forms are ideal in situations where there are no immediate next of kin, such as spouse or children.
